SERVICE COORDINATION

Established in 2012, the Service Coordination Division provides case management services to individuals with intellectual and developmental disabilities of all ages in 7 counties in Northwest Missouri. These counties are Atchison, Andrew, Holt, Nodaway, Gentry, Worth, and Harrison.

What do we do?

  • Service Coordination serves  310 individuals throughout our 7 county region. 

  • Each Service coordinator supports around 40 -45 individuals. Each individual has a person-centered plan that is updated at least yearly by the Service Coordinator.

  • The Service Coordinators meet with the individual and their team on a quarterly or monthly basis. During these visits, goals are discussed to see what services and resources are needed. From this, the Service Coordinator takes the needed steps to obtain the services.

  • Possible services could include: In-home supports, remote monitoring, respite, Day habilitation, Residential Habilitation, Employment Services, Behavior services, Specialized Medical equipment or home modifications, etc.

  • When the services are in place the Service Coordinator then monitors to ensure that the service being provided meets the individual's needs.

 Provide targeted case management services to improve the quality of life for individuals with developmental and intellectual disabilities. They develop a person-centered plan to identify current and future needs. This ensures independence by linking individuals and families to needed resources and services, advocating on behalf of individuals and families as requested, and monitoring services received to ensure the desired outcomes are achieved.

In addition to regular Service Coordinator duties, a Service Coordinator II assists with completing new individual service plans, caseload coverage, and training new service coordinators.
